Insurance
Our practice accepts most dental insurance plans, including PPO, Fee for Services and the Discount plan. We do not participate in any manage care including Medicaid, Medicare and HMO’s.
Payment Options
We offer Care Credit payment option and accept major credit cards. Payment is expected at the time services are performed. When more extensive dental care is necessary, financial arrangements can be made with our office.
Forms
We have provided patient registration forms for you to fill out at your leisure. Please bring them with you to your appointment along with your insurance card, state issued photo ID, and medication list. Completing the paperwork in advance will allow faster processing so that we can serve you more efficiently.
